We are required by our contracted insurance carriers to collect any appropriate co-pays at the time of service.  For your convenience we accept cash, Visa, MasterCard and personal checks.  A one time claim will be submitted on your behalf to non-contracted insurers as a courtesy (a copy of your insurance card is required to do so).  Any unpaid balances will be considered overdue at 30 days.

AFTER HOURS/URGENT CARE SERVICES:  A $100 deposit against your account is due at the time of service for all private pay patients, non-contracted insurance plans or for out-of-area residents.  This deposit is not considered payment in full and you will be billed for any additional balance in excess of this deposit or refunded if there is a credit.  You will receive an itemized statement with all charges for the visit .
